Admission Procedure
To enroll, kindly fill-out and submit the following requirements at the school or via email at [email protected]:
- Accomplished enrollment form [download here]
- Accomplished RFID application form [download here]
- Accomplished medical certificate (to be filled out by the child’s pediatrician) [download here]
- Photocopy of PSA birth certificate
- Photocopy of baptismal certificate
- Two (2) 1×1 inch ID pictures of the child (in white background) with name written at the back
- Two (2) 1×1 inch ID pictures of both parents (in white background) with name written at the back
- [Optional] Two (2) 1×1 inch ID pictures of the fetchers (in white background) with name written at the back
Age Requirements
Pupils will be assigned to their respective class by age and depending on the assessment of the school.
